Supported living

Supported living is a person-centred approach to housing and support that enables people to live independently within their communities. Unlike traditional residential care settings, supported living promotes autonomy, choice, and inclusion by offering flexible support tailored to each person’s preferences and needs.

We support people to lead meaningful lives on their own terms. Our supported living services focus on promoting independence, enhancing social connections, and achieving personal growth. We work closely with person to develop personalised support plan and review it regularly together with the person supported and the key people involved in their life.

It is about you

Person-centered care is the heart of a good supported living service. It means shifting the focus from what the agency thinks is best to what the individual being supported truly wants and needs. At Premier24care here’s how person-centered care translates:

  • Individualized Plans: There’s no one-size-fits-all approach. Supported living staff work with the person and their loved ones to create a personalized plan that reflects their goals, interests, and routines. This could include anything from managing daily tasks to pursuing hobbies or developing new skills.
  • Respecting Choices: Autonomy is key. Person-centered care empowers individuals to make choices about their daily lives, as much as possible. This includes things like choosing what to eat for breakfast, when to wake up, or what activities to participate in. Staff act as supporters, not dictators.
  • Strengths-Based Approach: The focus is on what the person can do, not what they can’t. Staff identify and build on the individual’s strengths and abilities, fostering a sense of accomplishment and self-confidence.
  • Communication and Collaboration: Open and honest communication is vital. Staff actively listen to the person being supported, understand their needs and preferences, and work collaboratively to achieve their goals. This may also involve working with families, therapists, and other healthcare professionals.
  • Flexibility and Responsiveness: Life is unpredictable, and supported living services need to be adaptable. Person-centered care means being responsive to changes in the individual’s needs, preferences, or health conditions. The support should adjust to ensure their well-being and continued independence.

Tenancy and support

Tenancy typically belongs to the person receiving support rather than the support provider in supported living arrangements. This means the person in our care adult holds the tenancy agreement for their accommodation, giving them legal rights and responsibilities as a tenant.

At Premier24care, we believe in promoting independence and empowering people to make decisions about their future and living arrangements. We support individuals to have a tenancy agreements that suits them. We aim to help them accessing long term accommodation with the necessary support in place.

Having tenancy rights gives people control over their living environment, including choices about their home, who they live with, and how they decorate their space. It also ensures that they have legal protections as tenants, such as the right to privacy, the right to challenge unfair eviction. We work with service users to ensure they understand their rights and advocate where we see fit their rights to move to where they pleased according to their circumstances.
